Ultrasound Therapy & TENS Combination is one of the most common treatments used in the management of soft tissue lesions, which make up most rheumatic problems. Although many laboratory-based research studies have demonstrated several physiological effects of ultrasound on living tissue, there is significantly less evidence for benefit in the treatment of soft tissue injuries. This may be related to a number of confounding factors, including technical variables, complexity and variety of underlying pathology in soft tissue lesions, methodological limitations of clinical studies, or true lack of effect. This review details the scientific basis for the use of therapeutic ultrasound in soft tissue lesions and the existing evidence regarding its clinical implications.

Physiological effects of ultrasound

Ultrasound can cause thermal and non-thermal physical effects on tissue. Non-thermal effects can be achieved with or without thermal effects. The thermal effects of ultrasound on tissue may include increased blood flow, decreased muscle spasm, collagen fiber distensibility, and inflammatory response. It is assumed that the temperature of the tissue is at least 5 min. Thermal effects occur with elevation to 40-45°C. Excessive thermal effects, especially seen with higher ultrasound intensities, can damage tissue.
