কোন ডায়াবেটিস মেশিন ভালো? রক্তের গ্লুকোজ পরীক্ষা করা ডায়াবেটিসে আক্রান্ত রোগীদের জন্য জরুরি। এই জরুরি কাজটিই স্রেফ এক ফোঁটা রক্ত এবং একটি ভালো ডায়াবেটিস মেশিন ব্যবহারে মাধ্যমে জানা যায়। গ্লুকোমিটার বা ডায়াবেটিস মেশিন থাকলে সহজে ঘরে বসে নিজেই কাজটি করতে পারেন আক্রান্ত রোগী। যন্ত্রটি হাতের কাছে রাখা ডায়াবেটিস নিয়ন্ত্রণ ও জটিলতা প্রতিরোধে খুবই গুরুত্বপূর্ণ।
বাজারে অনেক ধরনের ডায়াবেটিস মেশিন (গ্লুকোমিটার) পাওয়া যায়, তবে নির্ভরযোগ্যতা, নির্ভুলতা ও সহজ ব্যবহারের ভিত্তিতে কিছু জনপ্রিয় ব্র্যান্ড হলো:

কেন গ্লুকোমিটার ব্যবহার করবেন? তা নিয়ে অনেক কথাই বলা যায়। এককথায় এটি ডায়াবেটিসের রোগীর জন্য কিছু দরকারি তথ্য সরবরাহ করে। যা জানায়, তা রক্তে গ্লুকোজের মাত্রায় ডায়াবেটিসের ওষুধের প্রভাব। রক্তে গ্লুকোজের মাত্রার আকস্মিক পরিবর্তন, নিয়ন্ত্রণের মাত্রা, লক্ষ্যমাত্রায় পৌঁছানো হলো কি না। খাদ্য ব্যবস্থাপনা ও ব্যায়াম কীভাবে রক্তে গ্লুকোজের মাত্রাকে প্রভাবিত করে, তা আন্দাজ করা যায়। গ্লুকোজের মাত্রার নিয়মিত রেকর্ড রাখলে পরবর্তী সময়ে চিকিৎসার ধরন নির্ণয় করতে সুবিধা
হয়। অসুস্থতা বা স্ট্রেস (চাপ), হাইপোগ্লাইসেমিয়া ইত্যাদি জরুরি অবস্থায় এটি খুবই দরকার।
রক্তের গ্লুকোজ (Blood Sugar) কখন মাপবেন
- বাড়িতে গ্লুকোমিটার দিয়ে সপ্তাহে বা দিনে কতবার রক্তের গ্লুকোজ পরীক্ষা করতে হবে, তা নির্ভর করে ডায়াবেটিসের ধরন, কী ধরনের চিকিৎসা পাচ্ছেন এবং অন্য কোনো অসুস্থতা আছে কি না, তার ওপর।
- টাইপ-১ ডায়াবেটিসে আক্রান্ত ব্যক্তিদের দিনে ৪ থেকে ১০ বার (খাওয়ার ও নাশতার আগে, ব্যায়ামের আগে ও পরে, ঘুমানোর আগে এবং কখনো কখনো শেষ রাতে) পরীক্ষা করার প্রয়োজন হতে পারে।
- টাইপ-২ ডায়াবেটিসের রোগী যাঁরা ইনসুলিন নিচ্ছেন, তাঁদের সপ্তাহে কমপক্ষে তিন-চার দিন। এটা বিভিন্ন সময়ে পরীক্ষা করা যেতে পারে। কোনো দিন সকালের নাশতার আগে ও পরে, এক দিন দুপুরের খাবারের আগে ও পরে, অন্য দিন রাতের খাবারের আগে ও পরে।
- টাইপ-২ ডায়াবেটিসের রোগী যাঁরা খাদ্যাভ্যাস, ব্যায়াম ও মুখে খাবার ওষুধ পাচ্ছেন, এমন রোগীদের ক্ষেত্রে শুধু যাঁরা সালফনাইল ইউরিয়া (গ্লিক্লাজাইড, গ্লিমেপিরাইড) গ্রহণ করছেন, তাঁদের সপ্তাহে তিন-চার দিন (সকালে নাশতার আগে-পরে, দুপুর ও রাতে খাওয়ার পর) পরীক্ষা করার প্রয়োজন। অন্যান্য ওষুধের ক্ষেত্রে গ্লুকোমিটারে রক্তের পরীক্ষা আবশ্যক নয়, তবে সপ্তাহে এক দিন সকালে অভুক্ত অবস্থায় এবং যে বেলায় সাধারণত বেশি খাওয়া হয়, তারপর গ্লুকোজ মাপা উচিত।
- গর্ভাবস্থায় ডায়াবেটিস থাকলে সপ্তাহে অন্তত এক দিন সকালের নাশতার আগে-পরে, দুপুর ও রাতে খাওয়ার পর গ্লুকোজ মাপতে হবে।
- তবে ডায়াবেটিস খুবই অনিয়ন্ত্রিত থাকলে, অন্য কোনো অসুস্থতা যেমন-জ্বর, ডায়রিয়া ইত্যাদি থাকলে, কোনো কারণে স্বাভাবিক রুটিনের ব্যত্যয় ঘটলে, ডায়াবেটিসের নতুন কোনো ওষুধ শুরু করলে আরও ঘন ঘন পরীক্ষা করার প্রয়োজন হতে পারে। রক্তে গ্লুকোজ-স্বল্পতার (হাইপোগ্লাইসেমিয়া) কোনো উপসর্গ দেখা গেলে সঙ্গে সঙ্গেই পরীক্ষা করতে হবে।
গ্লুকোজের মাত্রা লক্ষ রাখা
কোন ডায়াবেটিস মেশিন ভালো জানার পাশাপাশি ডায়াবেটিসের একজন রোগীর অব্যাহতভাবে রক্তের শর্করা নিয়ন্ত্রণ জরুরি। কত থাকলে তাকে ভালো বলা যায়? খাওয়ার আগে প্রতি লিটারে ৪.২-৭.২ মিলিমোল এবং খাওয়ার ২ ঘণ্টা পর ১০ মিলিমোলের নিচে রক্তে গ্লুকোজের মাত্রা রাখা উচিত। তবে বয়স, অন্যান্য অসুখ, ডায়াবেটিসজনিত জটিলতা, গর্ভাবস্থা এবং আরও অনেক ক্ষেত্রে রক্তে গ্লুকোজের লক্ষ্যমাত্রা ভিন্নতর হতে পারে। এসব ক্ষেত্রে চিকিৎসকের পরামর্শ অনুযায়ী লক্ষ্যমাত্রা ঠিক করে নিতে হবে।
কোনো কোনো গ্লুকোমিটার মিলিগ্রাম এককে গ্লুকোজের ফলাফল দেয়, এ ক্ষেত্রে এই মাত্রাকে ১৮ দিয়ে ভাগ করলে মিলিমোল এককে ফলাফল পাওয়া যাবে।
গ্লুকোমিটারের ব্যবহার
বাজারে নানা মানের ও ব্র্যান্ডের গ্লুকোমিটার পাওয়া যায়। গ্লুকোমিটার মানসম্মত না হলে সঠিক ফলাফল না-ও পেতে পারেন। আবার এর স্ট্রিপ (যেটিতে রক্ত ফেলা হয়) সহজলভ্য কি না, সেটা জেনে নিন। কোনোটির ব্যবহার একটু জটিল। আপনার জন্য কোনটা সহজ মনে হয়, তা-ও দেখে নিন। এ জন্য ব্যবহারকারীদের জন্য নির্দেশিকাটি ভালো করে পড়ুন।

আরও কিছু পরামর্শ
- ভালো করে সাবান দিয়ে হাত ধুয়ে শুকিয়ে নিন। অ্যালকোহল প্যাড বা জীবাণুনাশক দিয়ে আঙুল পরিষ্কার করে নিতে পারেন, তা পুরোপুরি শুকানো পর্যন্ত অপেক্ষা করুন।
- গ্লুকোমিটার বা ডায়াবেটিস মাপার মেশিনে একটি পরীক্ষার স্ট্রিপ প্রবেশ করান। গ্লুকোমিটারের মডেলভেদে স্ট্রিপ আলাদা হয়, তাই শুধু আপনার মিটারের জন্য নির্দিষ্ট স্ট্রিপ ব্যবহার করুন। নকল ও মেয়াদোত্তীর্ণ স্ট্রিপ ব্যবহার করবেন না। স্ট্রিপের কৌটা খোলার পর একটি নির্দিষ্ট মেয়াদ পর্যন্ত ব্যবহার করা যায়, এ
বিষয়ে নির্দেশিকায় লেখা তথ্য অনুসরণ করুন। আধুনিক গ্লুকোমিটারে স্ট্রিপ প্রবেশ করার পর স্বয়ংক্রিয়ভাবেই তা চালু হয়ে যায়।
- সুই বা ল্যানসেট (যা একটি কলমের মধ্যে থাকে) দিয়ে আপনার হাতের আঙুলের অগ্রভাগ ফুটো করুন। প্রতিবার ভিন্ন ভিন্ন আঙুল ব্যবহার করুন। স্বতঃস্ফূর্তভাবে বের হয়ে আসা বড় এক ফোঁটাই যথেষ্ট।
- রক্তের ফোঁটা পরীক্ষার স্ট্রিপের নির্দিষ্ট জায়গায় স্পর্শ করুন এবং ধরে রাখুন। প্রয়োজনীয় রক্তের পরিমাণ গ্লুকোমিটারভেদে ভিন্ন (০.৩ থেকে ১ মাইক্রো লিটার) হতে পারে। খুব কম বা ছোট রক্ত হলে এরর আসতে পারে।
- মিটারটি কয়েক সেকেন্ড বা বড়জোর এক মিনিট পর আপনার রক্তে গ্লুকোজের মাত্রা পর্দায় প্রদর্শন করবে। মিলিগ্রামে হলে তা ১৮ দিয়ে ভাগ করলে মিলিমোল পেয়ে যাবেন। তারিখ ও সময় দিয়ে লিখে রাখুন।
- ব্যবহৃত সুই এবং স্ট্রিপ নির্দিষ্ট ময়লার ঝুড়িতে ফেলুন।
- নির্দেশ অনুযায়ী যন্ত্রটি নিয়মিত পরিষ্কার করুন এবং মান নিয়ন্ত্রণের পরীক্ষা করুন।
- চিকিৎসককে দেখানোর সময় গ্লুকোমিটার বা গ্লুকোজের চার্ট সঙ্গে নিয়ে আসুন, যাতে রেকর্ড করা গ্লুকোজের মাত্রা দেখে সিদ্ধান্ত নিতে সহজ হয়।
ডায়াবেটিস মেশিনের ভুল তথ্য
সঠিক নির্দেশনা মেনে ভালো ব্র্যান্ডের গ্লুকোমিটার ব্যবহার করলে সঠিক ফলাফল পাওয়া যায়। গ্লুকোজের মাত্রা অনেক কম (৩ মিলিমোলের নিচে) বা অনেক বেশি (৩০ মিলিমোলের বেশি) হলে গ্লুকোমিটারে সঠিক ফলাফল না-ও পাওয়া যেতে পারে। সে ক্ষেত্রে লো বা হাই নির্দেশ করতে পারে। ব্যবহার সঠিক না হলে বা রক্ত যথেষ্ট না হলে ত্রুটি দেখাতে পারে। রক্তের হেমাটোক্রিট অস্বাভাবিক হলে কিছু ওষুধ ব্যবহারে ফলাফলের তারতম্য হতে পারে। ব্যাটারি নষ্ট হলে, স্ট্রিপ ও কোড না মিললেও উল্টাপাল্টা রিপোর্ট দেখাবে। কোনো সন্দেহ দেখা দিলে অবশ্যই ল্যাবরেটরিতে পরীক্ষা করে নিশ্চিত হতে হবে।
গ্লুকোমিটার ও ল্যাবরেটরিতে পরীক্ষা করা গ্লুকোজের মাত্রা ভিন্নতর হতে পারে কি, প্রশ্নটি অনেকের। গ্লুকোমিটারে গ্লুকোজ মাপার জন্য কৈশিক নালির রক্ত ব্যবহার করা হয়, আর ল্যাবরেটরিতে ব্যবহার করা হয় শিরা থেকে সংগৃহীত রক্তের প্লাজমা। প্লাজমায় মাপলে গ্লুকোজ রক্তের গ্লুকোজের চেয়ে সাধারণত ১১ শতাংশ বেশি হয়, তাই ল্যাবরেটরির ফলাফল গ্লুকোমিটারের ফলাফলের চেয়ে একটু বেশি হতে পারে। তবে আধুনিক গ্লুকোমিটারগুলো রক্তে গ্লুকোজ মাপলেও তা প্লাজমা গ্লুকোজের সমতুল্য হিসেবে ফলাফল প্রদান করে, তাই এ ক্ষেত্রে প্রায় একই ধরনের ফলাফল পাওয়া যায়।
